What is the minimum information that should be included in a nurse's report to the nurse of the next shift about the patients?

Name and age of pt
Dr
diagnosis
activity level
if on I&O, what it was for your shift if so, foley, continent or not
diet, if diabetic and getting blood sugar tested, what the levels were.
lab values such as PT/INR, Na, K, ect
code status, if applicable
allergies, if any
IV's , if any
if any wounds, what sight looks like, dsg changed or not, amt of drainage.
Any changes in status (vs, level of consiousness, etc.)
Anything else that you think is important or that you would want someone to tell you!

I am not a nurse yet, but I would assume that it would be a summary of the treatments given to a certain patient in the past 24 hours...or at least since the reportee nurse has been on shift before.

If it is a new nurse, I would think that the reporting nurse would give a summary of the condition(s) of the patient, meds, treatments, etc. before the new nurse took on care.

Like I said, I am not a nurse yet, and haven't been present at a report...but I am assuming that's what goes on...makes sense to me. :-)
