In High School, we had an assignment to immitate a famous painter..I chose Andy Warhol (im a big Marilyn Monroe fan). Well, my art teacher did the silk screen for me & id love to know how she did it, what materials she used for it & what surfaces I can use it on.. (besides paper & t-shirts)
Also, can I take a photo I took myself & convert it into a silk screen to put on a shirt? Now......How do I do that?

There are many ways to create a image for a screen how she did it depends on her knowledge on graphic arts. She may have used a stencil, a cut out, She may drawn free hand on a clear material, or if she had access to a computer she may have used a graphics program.
You can print on just about any material you can find. Of coarse you must what type of ink to use which takes quite a bit of knowledge or at least access to a person that has that knowledge.
Yes, you can take a photo of yourself, only you know if you are capable of doing color separations and cropping and etc.. that are required to do the color separations for four color process printing. If you don't know what I am talking about forget it because man oh man it gets complicated.

Even a halftone in gray-scale is hard to produce correctly if your tension is off from screen to screen or you create more screen stretch by applying to much pressure from screen to screen.

I am sorry to say that your question does not have an answer Silk Screening or screen printing as it is called today is at trade, something that must be learned from trial and error or a teacher.

Silk screen printing works with stencils, so if you wanted to print an image of yourself (andy warhol style) you would probably need to simplify the photo into 'block' shapes, make stencils of each section, and print each colour individually.

As to materials, she probably would have used printing ink -but I don't know about using different surfaces (other than paper and fabric)

You know, your teacher would probably be very happy to tell you how she made it. She might be your best bet for doing something like that in an affordable way or atleast be able to point you in the right direction. She would probably appreciate your interest in the subject.
