List it. Especially if the job experience is relevant to the job you are applying for. The more stuff you have on your resume the better you look.
It doesn't matter how you were paid. Most employers will ask before they call old employers. You don't have to list the supervisors name and phone number on the resume. That's actually something for a reference sheet.
By the way, never list someone on a reference sheet unless they told you personally that they will give you a GOOD reference. Some people will say "Yeah, you can use me as a reference" but never clarify whether or not they will be a good reference. A bad reference is worse than no reference

It would depend on whether it was useful to the job I was applying for. If there was some relevent or related skill involved I might put it on there but it would depend on why it was only for a short time. The amount of money I'd earned would not be a consideration at all.

I would include it only if it had revelance to the position that you were applying for. If it was a seasonal or temp position then make sure that fact is included in the description of duties. Tho if you were fired or laid off then don't bother
The past employer shouldn't get upset with inquires

Any employment under 3 months isn't going to reflect negatively on you. Don't list it. You're not required to inform them of the employment. After looking at another question of yours, on the other hand, if you have been working for the temp agency for awhile, you might want to use them as a reference, but not the specific companies the temp agency assigned you to.

I would not put it because it was so short term. It would like you may have been a flake and they dumped you.
