It's okay to send food back under the following circumstances:
a) if it's not cooked as you ordered it (i.e. it's well done and you wanted rare)
b) If it's not what you ordered (i.e. you ordered steak and mashed potatoes and received steak and fries)
c) If it's bad/rotten (i.e. bad fish)
d) If it was misrepresented on the menu (i.e. it said grilled chicken breast and you got a breaded and fried one)
And sometimes it's okay to send back if you absolutely hate it - although be prepared (and even offer) to pay for it anyway. A good restaurant will usually allow you to order something else.
When sending ANYTHING back always be pleasant and courteous to the wait staff (after all it's not usually their fault).
Most restaurants want you to be happy and want you to return so it's not a problem to send food back.

Never? Oh my! What about shrimp that has gone bad? What about a steak that's still bleeding when you ordered well done? What about them putting mushrooms on something when you had not expected it and you happen to be allergic to mushrooms?
I send it back if I feel it is dangerous to eat, or if it obviously is bad. If I see a cockroach, back it goes! Bobby flay be danged.

I will send food back if it is not prepared correctly, served to me cold, has a hair in it.
I don't experiment and order things off the menu and then send them back when I don't like them.
I was a waitress for many years and I can tell you people send food back all the time and sometimes for really stupid reasons.
Such as they are half way finished eating and they want you to take their meal to the kitchen to heat it up for them again because it has gotten cold.
Idiots!

I would never send food back because you would never know what someone in the kitchen would put in it out of spite.
My two sons used to work in a restaurant years ago and they have told me how if the cooks were mad about the food coming back the cooks would put anything from body hair to dirt and spit on the food. No I would never complain about anything about the food.

Even though I have very much wanted to, I have never sent anything back. It freaks me out. Mostly because I am a very mistrustful person. It is unfortunate that people pay for a service, then have to be worried about getting a sneezer if they send it back because it isn't what they ordered or is cold.
Whoever is cooking it should be making it right the first time. If it isn't, what right do they have to spit or sneeze or whatever they do to it? It's their fault in the first place.

If I order a steak rare and it comes out well done. If the food tastes like it is sour. If something doesnt taste the way I expected it to, that is not a reason. Also if I have specified that I do not eat something (onions, milk products due to allergies). A good restaurant caters to its customers.

I never send food back, I guess unless I got the completely wrong dish. Like I'd ordered shrimp and a steak showed up. I can deal with most things though, like if the dish arrives with fennel even though the menu didn't mention it.

I've worked in the restaurant industry and as long as you are nice about it, it's fine to send it back if it's the wrong order or if it is undercooked or overcooked. No reason you should suffer for someone elses mistake.

It would have to be an extreme situation for me to send food back! The food would have to be so terrible it couldn't be eaten, and even then, I would have to think hard about sending it back. I have waited tables before. I never did anything to anyone's food, but I know how easy it would be for someone to do it.
