Address Tony Blair simply as - Dear Prime Minister, etc.
You may prefer your letter to be read by his 'personal private secretary', in which case address it to:-

The Prime Minister
10 Downing Street
Westminster SW1
Then begin your letter as above, Dear Prime Minister, etc.

In a letter, if it were a formal document, it would be "The Right Honorable, the Prime Minister"..........and the address of the House of Commons, or his official residence at 10 Downing Street
In fact, it would not be considered out of form for an ordinary correspondent to address him as "Mr. Prime Minister", or "Prime Minister Blair", or "Mister Blair". They are far less formal than in past years.

Not sure of the exact salutation, but it should be possible to look it up - many dictionaries, almanacs and other references have this information. According to Wikipedia, the Prime Minister is referred to as 'Prime Minister' and would be addressed in person as 'Mr. Prime Minister' - but any formal letter would be addressed simply 'Dear Sir.'

The PM on that last one stands for Prime minister. The PC stands for Privy chancellor, which is indicated by the "Right Honorable"

They all mean the same thing, but that's how I would put it. Formal, but not that that long.

you can adress him (in a letter) as Mr T Blair you would only call him sir if he's been knighted but you can call him sir as a mark of respect it depends what the letter is about
