He told her in the 100th episode, but Lex could tell she knew something, he was drunk and chased her and she fled, and her car was T-boned by a school bus. Clark ran to the scene and she died. Jonathan Drove by teh scene on route to meet Lionel at the Farm

Clark went to the fortress, and Jor El let him rewind the day. On take two he didn't tell her and the events of the day shifted and Jonathan had a heart attack after Lionel revealed that he knew.

Rumors suggest that Lana's baby(hyped in promos, so not a spoiler i hope) might be Clark's and if he thinks there is a chance he better tell her.

As of the Doomsday arc of the 90's Lana is one of the people who knows the truth, as she and the Kents visit Lois in Metropolis after Clark 'dies'. however, to confuse things, the lana of the comics was essentially split into Lana and Chloe for Smallville, and Chloe now knows.

Clark actually did tell Lana the truth in one episode and then he asked her to marry him. She said yes and then through a series of events ends up dying in a car accident. Clark goes to The Fortress of Solitude to ask Jor-El for help. He gives Clark a do-over and Clark decides for Lana's safety he will not tell her his secret because he has seen what it will lead to. That's why he broke up with her.

Well he did in the last season the only problem is SHE DIES! so he can never tell her and in the past superman (Christopher reeves) he tells Lois Lane but he makes her forget cause it's just too painful for her to bare with his secret soooo basically I don't think clark will ever tell Lana because it's not in the comics lol

I believe he refrained from it, because in one episode, he did tell her, but then everything went downhill and people ended up dying...so Clark got the chance to go back in time and redo it all, this time he didn't say anything.

I think that's how it went.

But yes, whenever I watch the show, I always yell at the TV, "Tell her already, damnit!"
