Once about 15 years ago. I got a $50 gift certificate and my picture on the wall with the saying employee of the month.

Yes I was at my former job.I got a free meal aday for a month and EOM Parking,but they took the spot away while remodeling the entrance to the building I still had to park in the main parkinglot.
