Cara M got it exactly right. If you're nervous about a pap smear then wiggles your toes while they are doing it. That always helps me to take my mind off of it. And also www.whattoexpect.com website is really helpful. They have message boards and alot of info about it all on there. ~Congrats.

No idea on genetics, but the first appointment can vary depending on the office. It could just be an informal meeting to discuss family histories, or it could be an all out exam. I hate exams.
They do a breast exam and a pap smear. Sometimes they will do the first blood lab then (they take quite a few vials for all the testing), or they may do it another day.

It varies from each office to another. I was expecting much more, as I saw in the What to Expect When Your Expecting book.
I had a 45 minute talk with a nurse, where I got a TON of information, magazines, ect. She took medical history and answered my quesitions, ect.
Then I saw my Dr. we already knew my due date as I'd had an ultrasound a few weeks earlier. He did a pap and internal exam, asked if we had any ?'s went over the issues we'd have with twins, and some far off thoughts to consider. (like a c-section, bedrest, ect simply because of twins)

I don't believe they are genetic. It's just when the egg happens to implant before it gets to the uterus. Don't quote me on that one for sure...
First visit they will give you a pregnancy test (urine most likely), they will check vital signs, including weight, ask you if you have any questions and give you prenatal vitamins.
They may also give you a pelvic/pap depending on when your last one was, and they may draw blood for normal testing.
Good luck to you!! You will probably also be given a bag of hand outs and things you may have questions about... I even got a book.

they'll confirm your pregnancy with a urine and blood test. Take your vitals, do a pap smear and internal exam to see how far along you are and predict a due date. Depending on how far along you are, they may schedule you for an ultrasound. it's nothing you haven't been thru before if you go to an OB/GYN regularly.
