and how many violations do you need to get banned?

I'd like to know, too.

However, I think that it increases your likelihood in getting banned if you call attention to yourself by responding to the message to defend yourself-- this gets personal attention, and if they look at your profile and see that you have a large number of questions and answers that violate policies (such as spam or consistently using abusive language), they'll pull the plug.

Unfortunately, I don't think each report gets investigated by hand- which is a pity because that would help eliminate spam accounts. With some of these violators, you take one look at their profile and can tell they need to be deleted. With other violators, if you look at their profile, you'll see that they really do want to participate and ask legitimate questions, but just occasionally screw up. Those accounts would be left alone.

I would think that banning would not be an automatic process. Perhaps humans investigate the user if enough violations are reported. The trouble with automated banning is that users WILL report others for things that really aren't violations. I really think that they must do it by hand.
