Great Question! I believe it can refer to both. I have had a great loss in my life that led me to be decieved about life.
Ok, When my twin died it was so shocking that life was like the Wizard of Oz...Toto exposed a mere man behind the curtains...Nothing but Illusions. I had lost a loved one very dear and beloved to me...my whole world... and been deceived. It was sooo Heart Breaking I saw no purpose to be here with the silly illusions anymore either...but there IS a purpose for me....so, Happy New Year. I just miss him.

I guess both a broken heart can mean anything and everything to a person but each person is individual so yeah it can be said a broken heart is refered to lost a loved one our it also can be a great deception in life in general
just what you believe it is thats all what matters

I think a broken heart refers to the loss of someone you love and not a delusion of of events in life that doesn't work out.

To me, it refers to anything that you have put your heart and soul into, and then you were let down and hurt by it.
It could be a love affair, or it could be the killing of 1,000 more wales in Japan, after you've fought long and hard to prevent that.
