For the first question, I would have to go with guilt. The beating of the heart follows the main character everywhere he goes after he kills the old man, just like guilt would. As for the second question, the protagonist is the main character in a story. In "The Legend of Sleepy Hollow," who's the main character? Ichabod Crane, if I remember correctly.
