I finished nursing school in 1997 and the best care plan book I used was:
Medical-Surgical Nursing Care Plans: Nursing Diagnoses and Interventions
by: Marie S. Jaffe

It was divided by Medical Diagnose first and then listed the common Nursing Diagnoses for that medical condition. Because I was new to nursing diagnoses I found this made forming my care plan much easier. Nursing instructors hate this book because it dose list the medical diagnoses first. But when you are new to nursing diagnoses it really helps.

Lippincott has some great general nursing books available. Also, consider online information through Ovid - great nursing references. Lippincott is available at Barnes&Noble or Amazon.
