Many, MANY, men like that look in a woman, dude! So, yes, they probably realize that she is a transsexual too.
As for question #2. No, Maybe, Yes! Who knows? Why would you care is, perhaps, a question to ask yourself.

she's post-op though so technically female now ... so they're not gay. she just never had facial feminization surgery so she still looks a little manly. this can be avoided if estrogen is begun only before puberty for a MTF transsexual
