If it a standard issue plate, odds are that they will simply reissue you new plates, not make a new plate based on your old number. You will have to bring in your old plates for new ones and pay a fee for both the plates and for the registration stickers (if applicable).

Go to the DMV and ask for a new one. They have to change the registration and new plate and void the old one in your name. They may charge you $10, depending on the state.
