I agree with what you listed; however, it's not necessary to have all the items on your list.

In order to be accepted at a top college, leadership has become increasingly important. It's not enough to join clubs at your school - you should also hold a leadership position. This shows that you can take initiative and that you are responsible, creative, and someone who can make a difference. Some students have the misconception that joining every club in sight will get them into a top college; however, some colleges may be turned off by this obvious resume-bulking.

Colleges also want to know who you are as a result of the experiences you have had. How have you dealt with challenges? What do you love to do or study? How have you contributed to the activities you have participated in? What are your values? How do we know that you are person with passions who will succeed at our institution?

All the areas that you mentioned:
Grades--were more difficult courses taken, such as math and science courses?
Community Services--were you really involved or just coming up with a list of fifteen-minute assistance?
Awards--were there any significance to the awards?
Merits--were there any relationships to educational goals?
Athletics--were there any outstanding events as a team player?

Others that you left off:
Clubs & Activities--were there any examples of leadership?
Work--did you have to help with family finances?

Very important--make sure you proofread your application. Poor grammar will hurt you!
