Is the only one of the above that a native speaker would ever say.
Most of the others are ok grammatically but are not used idiomatically.
The above is simple, direct, and commonly used and understood.

can i smoke? .. if your in a situation where it would disrespect them..
anyway you say it will get the same response.. there are just times when the wording is appropriate to the situation

both are correct. it depends on the situation. when you use do you mind my smoking that means you are already smoking. would you mind my smoking means that you are about to smoke.
