this sentence: Have you ever made a decision that you knew was wrong the minute you made it but was too late to change?

means ( if you have), you made a decison and after that , you realized it was wrong but it was too late to change??

You got it. Your explanation is exactly what it means. But you didn't just realize the decision was wrong after making it, you knew it was wrong AS SOON as you made it. That adds a little more drama to it.

Yes - it means that you made a decision, and realised that your decision was not the right one, but you can't take it back.
For example, if you were going out with friends and decided not to go, and then realised that you wanted to go, it would be too late to change your mind, because they have already left.

you got the meaning right. but to elaborate first the decision is made, than the decision was carried out, the minute it was done you realized that it was a wrong decision but it was too late to change it cause the decision was already made and carried out.
