I think she may have sensed her sister's magic or that the heads had a different 'feeling' to her son. Or possibly they have different spirit energy. Magic (in fiction) often seems to leave a residue that people can follow or such like.

It's been a while since I've seen the movie, but when Haku tells Yubaba that some precious of hers is missing, she looks at her baby (since her baby is most precious to her). At the time, her "baby" was eating the treats in the boxes aggressively. Overall, Yubaba saw how different her baby was acting, and that was when she realized it.
That's how I look at it.

Sure she realise that, because the thing that preciuos to her is money (gold) and her baby. When Haku said that she surely think that two things. The baby itself acting wierd not like usually he do.
