For the most part it could just be genetics but working in the medical profession over many years I will also tell you that a Dr will want to know more when he is presented with a patient with inverted nipples as it could be a sign of cancer.
If you have inverted nipples you probably ought to speak with your family physician to make sure it is a genetic thing verses a cancer thing.

Genetics wise, it's because the milk ducts are too short and are holding the nipple down too tightly. That's why breastfeeding or sometimes breast shells can cause the nipple to evert by breaking or loosening the adhesions.
