For mildew, just put lemon juice on it and let it sit in the sun. It really works.
As far as stains go, each type needs to be treated differently. You can try pre-treating with a commercial stain remover, but never dry the item in a dryer - air dry only. The dryer will set the stain forever. If it's air dried you have another crack at removing the stain.

bleach gets rid of mildew and a chelating agent like x-14 gets rid of mineral deposits. Dynamite gets rid of both!

It depends on where the mildew is -- but if you are talking about a bathroom or something, I highly recommend Kaboom. Nothing works better, in my opinion.

Odoban is a midewstat. It is great on stains in carpet, on hard surfaces, etc... It has a pleasant eucalyptus scent, too.
