What if you have a dad that is a twin (identical) and they married twin girls (identical) and they both had twins. Would the twins look alike even though they are not brother or sister?

Even if the second generation of twins were identical, they still would not be identical to their cousins. This is because the alleles (different forms of the gene) present in sperm and eggs are RANDOMLY segregated at the time of production of the eggs/sperms (gametes). In addition, after the fertilization of the egg by the sperm, many of the genes for appearance are not simply dominant or recessive, but will cause blending of the randomly segregated alleles.

So, it is certainly possible that identical twin double cousins would have similar features (such as blond hair, blue eyes, button noses), but they would NOT be genetically identical to their cousins, nor would they be physically identical.

The chances of that happening are slim, since twins skip generations. The identical twins' children would be the ones to have the twins next, not the twins themselves. And that doesn't mean that the next set of twins would be identical either.
