It's just a greeting, a substitute for "Hello".

It's not really a query about your state of well being.

I'm an American, and it bugs me too.

Just a plain, simple "hello" or "hi!" is really all that's needed.

I agree "How are you doing" isn't perfect English, but very few things are.

It's just a general question about how your day is going (or doing) or how you are feeling (or doing)

It's basically summing up any and all verbs into "doing", so the question can be used and interperted in a number of manners.

It is a greeting like 'hello'. They are being polite. Just reply " I'm fine " will do.

It's usually just a general greeting, most don't care so just say "fine, thank you, and you?"

They are just being polite. It is a pretty common expression "How are you doing today?" "How are you?" Just smile!

Because it's a normal habit. We see someone, we ask, "How are you?" and you answer "Fine, how are you?" or "Not doing too good. You?"
