No, however, anytime you use the hard drive it adds a little wear and tear, as with all mechaicanical devices. Formatting actually helps your drive because it helps to prevent corrupted sectors, and a corrupted boot sector.

It's a mis-concept !!! formatting a hard drive is no different to daily usage of a hard drive. All it does is clearing of the so all file system table which keep the references to all your hard drive data. However, there are different levels of formats that are different to a normal quick format which is available during the operating system (OS) installation. I'm sure 99% of the time people use normal/quick formats these days... Formatting and reinstalling the OS is not the solution for viruses. Maybe you want to look into ways of preventing virus infections and also use a good antivirus. There are good freeware antivirus out there... but if you have real problems with viruses, it is always recommended to go for a good paid anti-virus product over freeware. I think the money is well worth it given the hassle of formatting a drive every 3 months :)

Probably not noticeable. Formatting writes from end to end of the disk without a lot of head thrashing. An hour of formatting is less detrimental than an hour of intensive random IO.

every time you write to the platters you are affecting the structural integrity of the drive to some degree... but who cares. You bought the drive for a reason.

If done too many times consectively, then yes. If not, then no. In fact, It may ensure the life of your drive (if you have everything of importance backed up first).
