Soaking in cold water will reduce inflammation & pain.
After that, you should use the RICE method...
R.est
I.ce
C.ompression
E.levation.
I would recommend you use ibuprofen (motrin) for pain, but make sure you have food in your stomach before taking it.
Be careful when dressing your feet with sox/shoes.
Do not rip the nail off; simply bandage it with clean, dry dressing & when it's ready, it will either fall off, or you may need to trim it off. Don't do that now, as the toe is too tender.
If skin is broken, you may want to soak in epsom salt or use some Triple Antibiotic Ointment with pain relieving ointment.

You should trim as much of the nail as ppossible (just so it doesn't catch on anything and get torn worse). Then clean it well and use an ointment or cream like Neosporin or Bacitracin to prevent infection and keep it covered. Change the dressing once or twice a day at first, then only once a day. But use an ointment so it stays moist and does not stick to the dressing.

I think you should try to keep the nail on as long as you can to protect the injury. If it falls off later, that's fine, but you need to keep the toe dry and protected from bumping it again. Dryness will allow it to heal faster, so if it is clean, you don't have to clean it every 2 hours. Hope this helps.....

You should see another doctor and ask for a referral to someone who can help you. A physical therapist will be able to teach you exercises to stretch our the backs of your legs so you can maintain a normal standing posture. An orthopedic surgeon might be necessary if you have some deformity that prevents you from standing balances on the bottoms of your feet. Over time, without intervention, any postural deformity you have from walking exclusively on your toes will worsen. You are deforming your ankles and legs. You are not properly balanced. You are going to be more likely to fall and hurt yourself, or the posture itself will eventually throw your back and all the organs in your torso out of alignment. This is NOT a "natural dancer's" posture. Dancers who dance on the balls of their feet do an incredible amount of exercise to compensate--much of it is training to press down on the floor with their heels to reverse the stretch of standing high on their toes. There is a yoga pose called Mountain which is just a standing pose, in which you stand flat footed and explore the sensation of standing balanced on your feet. You might find some written descriptions of this pose online or in library books and begin regular practice of standing on your feet normally. Other postures will help you stretch out the backs of your legs. There is nothing wrong with moving fast on your metatarsals. It's what ballet and high heels are all about. You just don't want your feet to be stuck like that permanently. You really do need to stretch out your heels and calves and learn to keep your leg muscles balanced.

I'd say go to either Urgent Care or, if there is no urgent care in your community, go to Emergency. They will treat you and worry about who's going to pay later. The main thing you want to avoid is getting any sort of infection. That would be very bad. Go to Emergency. Get yourself treated by professionals.

clean it with rubbing alcohol or hydrogen peroxide daily. Cut the toenail off. keep your toe cleanly bandaged and make sure it doesn't get dirty. It should grow back eventually.
