OK, here goes the short version.
any amendments must pass both houses of congress. (senate and & congress) usally Bills (proposals of law additions or changes) are initiated by house rep from a particular state or states. it then goes to the senate and once revised and aproved it goes to congress. they can strike it down and give it back to the senate for revisions before a vote is made on it. once it passes congress it is sent to the pres for it to be signed into law. if the pres vetos it. it can be still made into law by a 2/3 vote by both houses.

The closest answer that is correct is A: 2/3 vote of each house of Congress. But that is not the only answer. Amendments can also be proposed by a convention called for by 2/3s of the states, too. The answer C which you provided is not exactly accurate.
