Hmmm....
Try starting with something less than "it's a date". Maybe she is going out to the mall, and you can meet her there? Maybe she's into some sports stuff that you can do together. There's always the "see you at the party" bi to break the ice.
The problem is, most guys are afraid of rejection (unless they're jerks). Just face the fact that she may or may not end up liking you, it's nothing personal. After all, you know dozens fo girls, and you're hung up on her, not some other one. She may also be hung up on someone else, not you. It's not because you're stupid, or ugly, or whatever excuse you may beat yourself up with. You're fine, you're just not for her.
The way to get her to like you, is to be yourself. Be nice, be friendly, but don't overdo it or try too hard.

Well, just ask her. I know it's difficult because you're nervous, but what's the worst that could really happen? She'd say no. And it's not like she'd seriously hate you for it. I mean if someone you didn't really like went up to you and asked you the same thing, you wouldn't totally resent them for it, would you?

Send her a card or a gift from a different name, and when you feel you've woo'd her enough, send another card to meet you in a certain place. It's not that hard to face someone, but I can understand if you possibly fear rejection. Or just confrontation.
