I had Lasik in my right eye (its the only eye I needed work on) in 2000. It was painless. Afterward, I kept my eye moist continuously for about 10 days and used antibiotics drops. It went great; I had no problems. I think that most of the people who have trouble afterward do so because they do not follow the post operation eye care instructions. If you have Lasik, you must follow those instructions. I now have better than 20-20 in my right eye. I also had a friend who wore soda bottle glasses (thick), he had the surgery and now sees great. I recommend it. Just check things out first, and get a good reliable doctor.

Because Lasik is a relatively new procedure, there haven't been any long term studies (10-20 years or more). However, most indications are it's safe most of the time. Unless your vision is downright awful, i'd recommend sticking with glasses for the next couple years (plus, glasses are coming back into vogue, so hey, it might not hurt to keep them).

I don't know of any. I do know someone who had it done and was satisfied. I would talk to an ophthalmologist (not one who does the procedure) for an overall evaluation, considering such things as your cornea thickness, possible issues such as Fuch's dystrophy, the amount of astigmatism, and other such issues before proceeding.
