Buy a fill in the blank one from the office supply store and get it notarized?

You can do it on the cheap. But a lawyer won't charge all that much. It's a one time expense and this is a big deal. I'd call around and ask some lawyers what they charge. You can make them an offer if you like. Also, there's lots of low cost legal clinics that have real lawyers working there. I'd pay the extra for the piece of mind of knowing it's done right.

You can buy the kit at Wal-Mart in the aisle you get printer paper, blank labels ect...for less than $20, all you have to do is fill it out and take it to a notary to sign. Any bank has a notary and if you bank there they generally don't charge you but if not then the charge is between $10-50.
