The USA is a republic, which means it has no monarchy, but it has a president instead. The present incumbent of that role is George W Bush.
Queen Elizabeth 2 is the queen of the United Kingdom and the commonwealth, plus the commonwealth's dependencies.
Both countries, the UK and the USA haved governments as well.

No. Queen Elizabeth II is firstly head of state of UK and the Commonwealth. She is thus Queen of Canada. Any Americans who want to live in a country with HM QE2 as head of state, only need to move to Canada. Deal done.

Shanghai is listed as a "Protectorate of the British Crown" but most other former colonies gained independence in 1776 or in the British Raj, India, 1949. That was when Pakistan was broken off and became an Islamic state. Hong Kong was a British protectorate also but went with the Chinese govt in the 1990s.

Queen Elizabeth is queen of Britain and the Commonwealth of Nations. The USA is a totally seperate country and not part of the British Commonwealth although Britain and USA have very close ties.
