If you have a guarantee on the condom from the shop you purchased it, then you are alright, otherwise in the future, wear four condoms one over the other or wear a garden hose for that extra, extra protection!

Get her to the chemist and get the morning after pill. There is also a pill that works 72 hours later I think.
If you can't find that (not really sure that exist, someone told me about it) and it's too late for the morning after pill, she could have a coil fitted in, if it's up to 5 days after having unprotected sex, that would work too.

tell her to get the morning after pill to stop her getting pregnant. get both of you to an STD clinic to check you havent given each other herpes
learn how to put a condom on properly, although good on you for trying to use one.

This happened to my friend's boyfriend back in college. He cleaned it, wrapped it in a box with a bow, and gave it to her as a gift. She was so surprized and kept the baby. I wouldn't do this if this girl never really wanted to get pregnant ever.
