It is an idiom in American English to indicate that the member of a team in either business or sports not only failed to complete the task assigned to them, but they performed very poorly with the attempted task.

One example is when the fielder in Amercan baseball has a fly ball hit to them, they are suppose to catch it, not drop it.

It comes from cricket. In the USA I would imagine it applies to baseball as well. A batsman is out if the ball is caught off his bat. If the fielder drops the ball instead of catching it, the batsman remains in. Dropping the ball is an error.
