the make is a V6 I think 1994 GMC Sierra 4X4 also how could this have happened

uuum some of the other guys are right some are wrong, these are all right i own a auto shop. Your tranny(transmition) fluid might be low, Your reverse gear might be broken or gamed or might have grease in it, or u migh just need a new tranny

I had one that did that. I dropped the pan and found a 50 cent part that broke that moves it into reverse. I made one out of steel.

some automatic trannys have a vacume hose going to a module fixed on the out fringe of the tranny,, if it has come off or has a hollow in it , it incredibly is going to interfear with opposite and moving out of first. attempt this ,, positioned the shifter in fisrt , then manually shift it to 2d and force , if it shifts manually its the vacum hose.

Probably that it is toast. But it would help if you said what vehicle and what engine, etc you have.
