The guy is just smiling back so he won't offend the girl. It is polite to smile back at someone when they smile.

It means - that they are both, for some reason or another, happy!
Why they are smiling! Who they are smiling at exactly! is open to conjecture.
You paint a picture that is suggestive, but what you may not know that one or both wears an earpiece connected to either a phone or an MP3 player / radio and are listening to a comedy and or talking with someone through a mike you can't see.

It's a natural reaction that people do without thinking. You smile at somebody, they feel compelled to smile back as not to look rude. In other words, the guy you have a crush on doesnt like you.

It depends on who they are and whether one of them is you and you don't know the other person. If it is and you don't, then maybe there is a recognition or an attraction there.
