I have been nursing my daughter since birth and introduced the bottle at about 2 months when I returned to work. I still do both. For the past month or so she will pull back for no reason with the nipple still in her mouth. No distractions are around. I usually unlatch her to see if she's wet or needs something else, but then she screams and rears back as if she's still hungry. Any suggestions on how to stop this. My nipple is so sore, I'm nearly avoiding nursing her on that side. (She seems to only do it one side?)

Yeeouch!!! I remember that. I could never figure out why -- maybe she was trying to increase the flow (or decrease it? if it's coming really fast). Maybe she's playing!
I would detach her immediately, accompanied by some sort of rebuke "No" "Ouch," or whatever comes out.
Wait just a minute before putting her back on. Eventually she'll get the idea "Oh, when I do that, I am temporarily deprived of my food; maybe I should stop."
It worked for me, eventually, and it worked for biting, too.

They all do that from time to time---awful! You could support her head with your hand while she nurses so at least she can't pull as far. Also make sure her breathing is not being obstructed in any way--- particularly since it is only on the one side.
If it is really a problem, try the 'Pavlov-dog' method. Every time she does it, make a sudden noise which frightens or disturbs her...she'll come to associate the behavior with a negative reaction. If that is not enough, a little flick on her cheek with your finger helps--- I did that when they had teeth and decided to bite. Obviously, not enough to leave a mark or anything... just to startle her and know that if she does this, that will follow.
I remember once watching a young mom with a kid who looked to be more than two years old that she was nursing. He'd sit in her lap and actually drag her breast around while he looked from side to side in the room--- and she never flinched! It was very disturbing on many levels-- not the least of which, my own breasts ached just thinking of how it would feel!

Make sure she is getting a slow flow bottle when you are away. I found when I worked several days in a row my daughter would do this. The flow isn't as fast as the bottle and they get frustrated.
I know you said there are no distractions in the room but is the TV on? Are there noises in the other room? She's older now and they become very easy distracted. Try laying down or go in a darke room. Watch her, when she starts to play around remove her right then and there before she pulls.
Make sure your supply isn't decreasing. No new medications? Lots of water. Are you pumping enough at work?
One of my favorite recommendations is breast compressions. You can search online for "Jack Newman breast compressions". If its a flow preference this will help.
Good luck!

Yeah, I remember that. Then my babies would look up at me and smile, like they were being cute. Your baby is discovering her new mobility! She can move around and look at things. You're doing the right thing. You have to teach her that mealtime is not playtime. When she pulls off like that, unlatch her....every time. It's like when they start biting, you do the same thing. Then when she's ready to nurse, put her back on. If she pulls on you again, tell her "no" and take her off again. You might also try some different positions.

Man I use to have that happen all the time. I have nursed 6 babies and everyone of them did that. I hurts like crazy. I am not sure that there is anyway to make them quit except to let them know that it hurts and it is something that must not be done. Let them know that they are in trouble. I wouldnt spank them or anything but they knew they where in trouble. I would immediatley put them down. You could always just stop nursing them sence they are on the bottle too.

Good Luck!

OUCH! Maybe she isn't wanting the breast anymore as food and just using your breast as a comfort thing. I would try switching to the other side, or try giving her a bottle. Don't allow her to do that because she is just playing with you. If she keeps on doing it, take it away from her. Good luck though!

Lay down. Put a pillow behind her. She will have nowhere to go.

I hated that too.. nothing like seeing your nipple stretched to like 4" long
