The first is correct.

The second is incorrect because only the words attributed to the speaker go inside the quotes. The words describing the speaker's action (intoned or said) go outside (as in the first sentence.)

The third and fourth are a matter of style. Generally if the titles are part of a larger work, e.g. the science book or a book of short stories, then quotation marks are used. If the work can stand alone, say the title of a movie or book (or perhaps a short story published by itself), it would be underlined or italicized. I have seen mainstream publications do either.

The first sentence appears to be correct, since it stops and restarts the quotations for the section that was not actually spoken.

The second sentence does not do this, as it makes it appear as if the speaker also said the words "she said".

The third sentence is correctly quoting a book cover, and is fine for me, but some english teachers may tell you that the book title should only have had single quotes (') not double (").

And finally, the last sentence incorrectly contains the comma within the quotes. It was probably not part of her short story title, and so that is incorrect.

1. A, C 2. A 3. A 4. A 5. C Writing "The New York Times" is okay if that's the name of the newspaper. If it's just "New York Times" the "The" shouldn't be in quotation marks. Using single quotation marks is also okay but it has to be consistent. You can't use double and single for the same purpose.
