There are at least three different copyright dates (1991, 1993 and 1995) on Amazon, with a variety of prices. I think they're all listed as hardbacks, and one was limited.

I was thinking this title was familiar. I sell books on-line, but don't seem to have this listed. If I have it, it's buried right now. My apologies.
