Really none of the above answers will work totally.
What you need to do is to mix some liquid detergent with some Brush Killer and spray it on. That will not totally work the first time, so keep doing it.
Ivy has a wax on the leaves and the detergent is used to work on the wax.

your gonna have to strip 2-5 inches of soil it a pain in the a$* once you do that spray round up in the dug up area wate about 24 hrs and lay down some new soil
when iv had to do it in the past iv used a bob cat and striped the ground if you have to do it by hand pull it up and rotill the heck out ov the ground

You invite some sleezy girl over and "take her" up against the fence. Have her come over at least once a week... Then you will kill the ivy by smooshing it! =) OK, Carl?

Apart from digging it out - making sure to get all the roots, you can pour hot water with salt over the area. You will need to add new soil/nutrients when planting in this area in the future.
I've also used diluted Javex with water and sprayed them. They will eventually die out.

Go to the home and garden store and ask about what weed killer will kill the ivy for good. Once it's dead, you should probably dig it up and burn it to be safe.

There is a spray called Round Up that will kill everything it touches, ivy or grass, etc.

Buy a spray from like home depot or sumthin. My great uncle used it in my front yard and now its gone.

Pulling it out every time you see any is my suggestion. Sooner or later you should get most all of it.

you will have to pull and dig it all up and burn it or it will take root where you throw it down at
