Sometimes you can't. But the most obvious sign is that you feel more kicking at the top of your stomach rather than the bottom.

when you feel around down there with your hands it'll feel like there is something big and hard there. and then the top of your belly will have something big, round and soft stickin out at ya. my girl's been in that position for about 2 months now. so glad I'm almost due!

I have three kids, and yes, I was able to feel the 'flip'. But if you don't feel it, don't worry! Some babies don't flip until you're in labor. If you're feeling more movement toward your ribs, it's probably the baby's knees and feet, meaning it's head down.
