The same that happened in the year 2000.
Some predicted all sort of catastrophic events, but the year 2000 had a meaning only because we were using the Decimal System to count the years!

They believed things ended and started anew. And their civilization was at a level of competence at dealing with its problems that it worked out that way 3-4 times, more or less, as near as the current archeological record can tell. That is to say, the rulers ruled with a pretty heavy hand, generations accepted that, many cities lost out to others because they couldn't use the resources they needed to perhaps due to disaffection among their people, and the ones that limped into their 4th century of lasting had built up so many problems in their societies that they were ripe for collapse from within and, the coup-de-grace, since just about everyone in each city's hegemony believed the end was coming about then, things basically all fell apart with what looks like savage internecine bloodbaths leaving very few people in a subsistence environment to pick up the pieces and start new societies. Maybe this last time, they would have solved the weaknesses of their societies (perhaps very heavy-handed ruling combined with severe selfishness?) but we will never know as some folks from Spain who happened to be just about as brutal but with better equipment showed up and savagely ended the last cycle.
Will anything then happen when the calendar runs out again? Nothing of note. Their societies today are kept in check by the Mexican government. A bit of a resistance by the Zapatas is a far cry from any kind of set of forces grinding toward destruction of society in the region and a massive bloodbath leaving only a few thousand alive.
Does it mean anything to societies outside their region, like Art Bell looney-toons like to suggest? No. We got over that whole cities falling upon themselves and disappearing into the sands about 6,500 years ago and the skills to keep it that way have spread from Sumeria, et al., to most the rest of the world. So no, we won't be seeing our societies rent and torn by ourselves and shattered into disappearance.

Nothing unusual will happen. The Mayans did not and could not predict the future. Even the popular idea that their calendar ends on that date is not correct.
