It depends on the person.
There still are TTY's, but those have basically become obsolete, I don't know anyone 30 or under who still uses one. Most deaf people use one of three services...VCO (Voice Carry Over) IP-RELAY, or VRS, (Video Relay Service)....Deaf people often use their computers for these services, or TV in the case of VRS.
VCO is for deaf people who choose to use their voice and can speak clearly. The deaf person speaks directly to the hearing person, and an operator listens to the hearing person and types what their responses to the deaf person.
IP-RELAY is a text service where the deaf person types what they want said to the hearing person and an operator conveys it. Like with VCO, when the hearing person speaks, an operator types what they say to a deaf person.
VRS is a service done through the use of videophones. A deaf person calls an operator using a webcam/videophone and then sees an interpreter (in the US, translating for American Sign Language (ASL) and English...or now they have ASL and Spanish as well). The deaf person gives the interpreter the number of the hearing person they want to call, and both speak/sign through the interpreter.

There is a service called TTY and everything you say goes through an operator. The deaf person types on a special keyboard that is transmitted through the phone line - much like a modem in a computer. If both parties are deaf, with the required equipment, there is no need for an operator. If you are hearing and you need to communicate via telephone to a deaf person, you can look in your local phone book for the number to the TTY operator.
