For tax pruposes, do I document miles I drive to each business account or do I document the miles from my place of business to each account?

Document the miles you drive to each location. On the mileage log should be the miles it took to drive there, the company name and contact and the reason for the call. If you take them to lunch include on the lunch receipt who attended. This should cover all the bases for the IRS.

Miles to business account from principle place of business. Commuting miles (from home to office) are not deductible.

For the purpose of documenting the miles, start a log: Date, time trip began, client and purpose, beginning mileage, ending mileage, time trip ended, miscellaneous (parking, tolls, lunch/dinner, etc.)

Buy a cheap handheld voice recorder, use it to record the information, later transfer the information to your log. That way you don't have to carry the log with you, forget to make entries or remember the details of the trip.

Personally I would make a spreadsheet with starting and ending mileage slots on the sheet or go to a truckers website and order a milage book.
