Like everywhere else in life, Xbox live has it's mature and immature gaming crowd. Your best bet is to ignore it (like I do), or be a little more selective about who you play online with.
Since you can dictate who is on your friend's list, you can easily add people who you meet online that are fun to play against.
Or.... you can simply take off the headphones, and just play the multiplayer without them. You don't need to hear other people talk online to play on Xbox live.

There are trash talkers everywhere, it's not a big deal.
I would suggest using your headset so you can talk with your teammates. I play Halo 2 often Live and having teammates without headsets is very annoying.
