Honestly, I've been told that they look like large rocks of salt, they are bumpy and crystalline looking. While normal ovaries are shiny, soft and mostly smooth.

On cutting across the ovary, there are multiple fluid filled cysts seen at the periphery where the ova are present. Normal ovaries may have just one or two small cysts. The external view shows the cysts bulging from the surface of the ovary of varying sizes.

I have PCOS and normal ovaries are reddish in color. Mine are more pink and look like they are wrapped around by a string of tiny pearls. Most cysts are very small, like millimeters, however the ones that are problematic are centimeters and growing. Most cysts are controlled with birth control and prevent flare ups. However, some will grow and burst and cause tremendous pain. Most doctors will not remoce the cysts until they become a large problem.

Through ultrasound they are larger than average and have more cysts on them. Mine looked like big chocolate chip cookies. I also had a gyno tell me once that they felt larger while she was doing an internal exam. She grabbed a hold of one of them and I came off the table. Kicked her good. Needless to say I never went back to her.

They aren't smooth...they have little cysts or polyps on them. Kind of like looking at a picture of the moon or brain....you know, like smooth in some places and lumps in others.
