They are actually built inside the bottle with presicion instruments. I saw how they do it on TV.

They remove the bottom of the bottle to put the boat in the bottle, then seal in again

They make it with the sails folded up , slide it into the bottle and then pull a string and the sails open up. Or alternatively they make the boat and then have the bottle blown around the ship

First of all they build the hull. Then they attach the masts, only they are not upright but lying flat on the hull. They leave small strings attached to the tips of the masts which allows them to be pulled to the correct position once they ship is in the bottle.
