Nope, the original astronauts on teh moon survived. The average amount of radiation received on one trip to teh moon doesen't even equal up to a couple of medical X-rays. It's not much radiation. You would have to go much deeper into space to run into large radiation problems.

It could, given the right conditions.

However, it didn't happen during any of the moon missions or moon landings, and likely won't happen during the upcoming missions either.

If you're speaking of the Van Allen belt (one reason given to support the silly notion that we didn't go to the moon), no.

edit: also tho, if something like a powerful solar flare or a coronal mass ejection hit the traveler it would not be survivable.

Absolutely it would. Without some sort of protection/suit you would be dead in minutes and that is not including extraordinary solar events such as flares etc.
