Those are not ear plugs. They are inner ear monitors. Most of them are custom fit and can range from 100 to 3000 dollars. And by the way, a monitor is for the siger to hear his voice over the other instuments. If you will notice, some bands have small speakers on stage at their feet pointed back at them. With ear monitors, you don't need the floor ones.

From what I know, what the artists hear and what the audience hear during a performance are different. Artists may hear louder noise, and so on. Therefore, some use plugs to keep their hearings clear. The plug plays the performance's sound, but at a considerably lower and more acceptable level of noise. A musician, then, will be able to adjust his/her play according to the feedback from the plug. For example, if it turns out that she played the guitar a little bit too loud, she would have to readjust her play.

Nevertheless, really good musicians do not need the plug, because they can "automatically" feel the music and adjust their play with the sound they are making.

Most musicians know they like to hear. They don't all accept that the will eventually lose their hearing if they don't protect it. You could try to ask Peter Townsend of the Who, but you'd probably have to write the question on a piece of paper since he can't hear very well anymore.

Many time those are not ear plugs but are ear phones so they can hear only those instruments that are necessary to their part in the band.

Do you realize how loud that music is on stage? How loud their screaming fans are?

They don't want to loose their hearing, because it's kinda hard to make good sounding music if you can't HEAR it!
