Harare Central in Zimbabwe.
Prisoners are kept without shoes, little food and no health care.
Conditions are quite atrocious.
Historically, it has to be the S-21 prison in Cambodia operated by the Khmer Rouge from 1975-1979. Conditions were similar as in the above mentioned prison. In addition, all but 7 of the 14200 imprisoned died there.

This is in a BBC article titles "The worst prisons in the world" so I would hope and think that it's pretty reliable. This one is in South America. Here is a quote:
"more than 330 people were murdered inside Venezuelan jails last year. That is about six killings a week in the country's 35 prisons."
And more than 1,000 are injured from all of the violence!
Also, the conditions are terrible. Often, more than one person occupy one bed, and the inmates make their own crude knives and weapons. A while ago, they even found some HAND GRENADES there!
A specific name was not given but it does mention that there are several prisons there so I think it may just be Venezuelan prisons in general.
I'd go with with saying Venezuela. :-)
EDIT....
I just looked through some other answers and it seems as though another person answering found the same article from BBC that I found. ;)
