I know HTML and CSS and I know how to edit templates and I have my own site at http://colorblindcs.net I am here to ask, if I want to design my own template, do I start on Dreamweaver or PhotoShop? A guide or your own suggestion would be much appreciated. A more thorough and extended answer is welcomed as well.

Most of the designers start with Photoshop. They design the way they want the pages to look (structure, colors, fonts, size, images...). Then they put their designed page as trace image in Dreamweaver.
You may find the tracing image under:
Modify + Page Properties... + Tracing Image (last category) - you can modify the transparency of the tracing image to identify it form the real constructed page.
