Refer today Times of India. There is detailed article regarding Passport Application and Enclosures. 14 Documents they have mentioned for identity proof. If you provide any 3 of them will be sufficient.

For Passport address proof, BSNL phone bill is valid. Even Driving license they accept. If you are providing the phone bill, give the latest bill.

What country are we talking about? When I got my first US passport, I did not need anything for my address. The passport does not contain your address in it, and the address is only for the delivery of it to you. The same goes for renewal. They DO require a proof of your citizenship though.

look applying for passport has become much easier than ever before. now you have to submit only 3 original documents for applying to get a rebate for not having any police verification. so better you submit any from these documents like your phone bill, pan card, Voters' id card, ration card, driving license, etc...

As adress proof none of these are valid now. It was valid one year earlier. Now you need to submit residencial certificate. which is must now.
