It takes about 28-30 days to break a bad habit (such as biting nails...) and it takes about the same amount of time to establish a new habit (keeping counters clean)... Can you break a habit and establish a habit at the same time? Sure, if you don't take on more than you can chew. When we do that we get overwhelmed and "go off the wagon" and that's when resolutions aren't kept.
Look at your resolutions. Put them in order of importance. Figure out which one you really want to start with. Keep at it on a daily basis (in the case of "exercising more" -- make a schedule and stick to it -- do make-ups if life or work get in the way). Soon, that first one will be "established" and you can start on the next one -- but don't try to start the next one until at least 30-40 days later.

Only set yourself two or three resolutions. Ensure that these can be broken down into smaller steps. Steps that are achievable by certain dates - they have to be achievable. Set some deadlines for when you will achieve them by. Draw up a plan of action; with the resources you have, the support you need, deadlines, your strengths etc. And keep a diary, where you track your resolutions on a daily/weekly basis. Allow yourself some room for slip-ups and ensure you incorporate a reward for everytime you achieve part of them too.
I also find that writing down what you have achieved (just before New Year) over that year, can help to motivate you for the next year's resolutions.

The biggest problem with new years resolutions that everyone makes is. It is a resolution for the YEAR. The resolutions are made at a time of the year when your stuck inside your cubical freaking from phobias. Give the process the full year to be accomplished. Don't just say it didn't work two months latter, keep at it. Happy New Year my friend.

Actually, I set monthly resolutions. I pick a habit I want to start or break, and then decide what I'm going to do for just a month to accomplish that goal. If you can do something for 21 days, you're set and can continue doing it. It's hard to do a whole year, but a month-long goal has worked really well for me, and I've been doing this since the 80s.

One of your best strengths is to acknowledge your weakness. If you have never been good at setting and achieving goals then just don't set them outside your reach probability factor.
It helps to write them down but most people don't follow through and achieve them in superlative fashion.
I'm still working on my 1996 resolutions to do better on my resolutions.
I am obviously not a CPA or in a detail oriented profession and that certainly helps in my case.
Let us know how you are doing in this regard in a couple weeks.

Set a realistic resolution. Don't choose something that there is absolutely no way you can achieve. Make it something simple, that you really want to do and set little goals...little steps to take to achieve it. Making the mini goals will give you incentive to achieve the overall resolution.
